  1. obtain a medical certificate stating that there are no medical contraindications to driving (the medical examination for the certificate may be carried out at OSK SzAŁ by appointment with an authorised doctor).

It should include an image from the top of the head to the upper part of the shoulders so that the face occupies 70-80% of the photograph, showing the eyes clearly, especially the pupils, and depicting the person in a frontal position, without headgear or dark glasses, looking straight ahead with eyes open unobstructed by hair, with a natural expression on the face and the mouth closed.
    • a medical certificate stating that there are no contraindications to driving, issued by a doctor authorised to examine drivers,
    • a psychological certificate stating that there are no psychological contraindications to driving (for CAT C1, C, D1, D, C1+E, C+E, D1+E, D+E),
    • written consent of parents or guardians if the person is under 18 years of age,
    • a photocopy of your driving licence (if you have one),
    • a photocopy of the driving licence issued abroad with a translation (when exchanging a driving licence not specified in the road traffic conventions).
